The PlasmaWell Approach to Outpatient TPE

Choosing an outpatient therapeutic plasma exchange practice involves more than choosing a procedure. It also means understanding who will oversee your care, how recommendations are made, and whether additional treatments are appropriate for you.

At PlasmaWell, our approach brings together individualized treatment decisions, on-site clinical oversight, and nutrition support tailored to each patient.

TPE without a required bundle of additional therapies

At PlasmaWell, patients are not required to purchase a treatment package that combines TPE with exosomes, stem cells, peptides, or a predetermined supplement regimen.

We believe each recommendation should have its own rationale. Before adding a test, supplement, or treatment, we consider your medical history, current medications, relevant findings, potential risks, and the evidence supporting its use.

A treatment’s inclusion in a package does not establish that it is necessary or appropriate for every patient. You should understand what each component is intended to accomplish, what is known about its safety, and what remains uncertain.

Why we evaluate additional therapies individually

The safety and evidence behind additional therapies vary substantially.

The FDA has warned about serious risks associated with unapproved regenerative medicine products, including products marketed as stem cells or exosomes. These warnings should be part of an informed discussion whenever such products are offered.

Peptide products also require individual evaluation. “Peptide” describes a broad category; safety depends on the specific substance, formulation, source, dose, and intended use. The FDA has identified potential significant safety concerns and limited human safety information for certain substances used in compounded peptide products.

Supplements deserve similar care. They can interact with medications, duplicate ingredients in other products, or cause harm at excessive doses. A supplement being available without a prescription does not establish that it is appropriate for you.

Our goal is to make recommendations you can understand and evaluate, with a clear explanation of their purpose and limitations.

Functional nutrition support tailored to you

PlasmaWell’s team includes Aubree Steen, a Functional Nutritional Therapy Practitioner, or FNTP. Aubree works remotely under the clinical supervision of Ellen Gott, PA, contributing nutrition and lifestyle information and helping identify potential testing and supplementation needs.

When appropriate, we consider targeted laboratory testing alongside your symptoms, dietary intake, medical history, and existing results. Testing should answer a meaningful clinical question and help guide a decision. We consider the evidence, reliability, limitations, and cost of a proposed test before recommending it.

Our approach to supplementation is targeted and informed by relevant laboratory findings and clinical assessment. Recommendations may address an identified deficiency or another specific need, with attention to dose, interactions, duration, and follow-up.

The aim is to support your individual nutritional needs before, during, and beyond a TPE treatment course. We do not assume that every patient needs the same products or that adding supplements necessarily improves TPE outcomes.

A prescriber physically present during treatment

At PlasmaWell, Ellen Gott, PA, is physically on site during TPE treatments, alongside the RN providing treatment care.

This gives patients access to both nursing care and an on-site licensed prescriber who can assess concerns, review symptoms, and make clinical decisions. Our physician medical director is available for consultation as needed.

On-site clinical presence does not eliminate the risks of TPE or replace emergency services when needed. It is, however, a deliberate part of how we organize patient care.

When comparing practices, ask who will actually be present throughout your appointment, who can make treatment decisions, and how emergencies are handled.

Questions are part of informed care

Before choosing a TPE practice, consider asking:

  • Can I receive TPE without purchasing unrelated therapies?

  • What evidence supports each proposed treatment or supplement?

  • Who reviews my medications, testing, and individual risks?

  • Who is physically present during treatment?

  • How will we assess whether treatment is helping?

  • What are the full costs, alternatives, and reasons to stop?

At PlasmaWell, we welcome these questions. We want you to understand the reasoning behind your care plan—including the uncertainties—and participate in decisions about what is appropriate for you.

TPE has established uses for selected conditions; other applications remain investigational or have limited evidence. Individualized care, laboratory testing, and clinical oversight do not guarantee benefit or eliminate treatment risks.
